A Clinical Review for the Spleno-renal and Porta-caval Shunt



Abstract
Author, having reviewed 35 cases of portal hypertension admitted and managed by surgical intervention at the Department of General Surgery, College of Medicine B.N.U. during past 11 years and one month period from May 1963 to May 1974 and compared S-R shunt with P-G shunt about operative mortality, postoperative variceal bleeding, hepatic coma, 5-year survival rate and excellent quality of survival, made the results as follows:
1. Of 35 cases, 23 cases were operated on by the method of¢¥ the S-R shunt with splenectomy and -12 cases by P-C shunt.
2. S-R shunt was applied to the cases of extrahepatic portal vein obstruction, liver cirrhosis; with hypersplenism, liver cirrhosis with extrahepatic portal vein obstruction and portal hypertension with no demonstrable obstruction.
3. The rate of operative mortlity was 8.7% in S-R shunt and 41.7% in P-C shunt.
4. The rate of presence of variceal rebleeding at the lower part of the esophagus was 17. 4% in S-R shunt and 16.7% in P-C Shunt.
5. The rate of development of postoperative hepatic coma was 17.4% in S-R shunt, and 66.7%, in P-C shunt.
6. In 5-year survival rate, S-R shunt was 62.5% and P-C shunt was 20.0%.
7. Excellent quality of survival was 43.8% in S-R shunt and 20.0% in P-C shunt.
